Breast Augmentation and Breast Lift

One of the most common reasons women seek breast surgery in our practice is to fix the deflated appearance of their chest after having children. Women miss the fullness in the upper half off their breasts that looked good in clothing and bathing suits—the volume that mysteriously went away after breast feeding. Most of these women express a desire to increase volume or fullness, plus they want their breasts to be perkier.

What exactly does perky mean? Perkiness exists when the nipples are in an ideal position. This means they are above the breast crease and centered on the most projecting part of the breast. As your breasts enlarge from pregnancy and breast feeding, the skin stretches. When your size decreases, you are left with extra skin. This causes the nipples to fall from their ideal position as the breasts deflate.

How can we fix it? First, we need to decide how much more volume you want. By using breast implant sizers, we can pick the right implant volume. This is the most accurate way to find the size you will be happy with in the long term. Once you have chosen your size, we need to decide if you need a lift.

Whether you need a breast lift or mastopexy depends on your nipple position relative to your breast crease or inframammary fold (IMF). Basically, if your nipple is above your breast crease, you do not need a lift. If your nipple is below your breast crease, you need a lift. If your nipple is right at your breast crease, it depends. If your nipple is right at your breast crease and you choose a small implant, you will probably need a lift for the best possible shape. If you choose a moderate sized or larger implant, you will probably have a good shape without a lift.

The pictures below show examples of nipple position. Both women show signs of deflation where they could benefit from more volume, but their degree of perkiness if very different. The picture on the left shows a woman with good perkiness—her nipples are above the fold. The image on the right shows a woman lacking perkiness with her nipples below the fold.

In the next blog, we will compare the results when the perkier breast is treated with a breast augmentation and the non-perky breast is treated with a breast augmentation plus a breast lift. In part I, you learned how to tell if you need a breast lift with your augmentation to get the best possible shape. When a breast implant is inserted, its bottom edge is near the breast crease or inframammary fold. Adding a breast implant does not lift the nipple. So, if your nipples are below your fold before, they will be below your crease after surgery, too. That is, unless you have a lift.  What does it look like if your nipples are below your fold and you get a breast augmentation without a lift? Well, not good generally. Since the implant is sitting above the fold, its shape and volume are higher on the chest. The nipples and surrounding natural breast tissue, however, are lower and appear to be melting off of the implants. Not pretty. To summarize, breast implants CAN be used to correct deflated appearing breasts when the nipples are not too low. If the nipples are too low, a lift is necessary, too.

Now, let’s look at our before and after pictures from part I. The perkier patient had some deflation, and the breast augmentation filled out the extra skin, improved fullness and enhanced shape. The less perky patient had an augmentation plus a lift. The implants filled out the breast and improved fullness, but the lift was necessary to maximize shape. You can see that the nipples are above the fold now, and the shape is much better.

Both patients were able to get a nice shape and a result that made them happy. The trade off with a lift is more scars. When faced with severe sagginess, many women would gladly accept the scars for a much better shape. Fortunately, the scars generally improve a lot over time. In most patients, the redness fades and the scars soften. In fact, we take some extra measures in our practice to help these scars fade and soften more quickly with scar massage and a special scar cream.

A better bosom is no doubt desirable. With the various options available to you, however, it may feel a bit overwhelming and intimidating when trying to decide which procedure to get. With a bit of information about these two types of breast surgeries, they are easy to understand.

Breast implants are silicone or saline medical devices that are inserted either over or under the chest muscles to make the breasts larger. They usually look like small bags, with silicone implants coming pre-filled from the manufacturer, and the surgeon fills saline implants after their insertion into the body.

Implants augment, or add to, the breast and make them physically larger. This is beneficial to those looking for increased volume and a fuller bust. This could be because of a loss of volume due to breastfeeding or simply because you feel your breasts are not at the size you wish them to be. Breast implants come in many varieties of shape, feel, and size and when speaking with Dr. Vennemeyer, you’ll go over these options so that you both can ensure the result will be to your satisfaction.

A breast lift re-positions the breasts on the chest to a more youthful, perky place.

A breast lift moves the breasts up. Sometimes, due to age, pregnancy, or other factors, the breasts can sag and sit lower than their initial natural position on the chest. This can make fitting clothes, bras, and dresses difficult.

To lift the breasts, the surgeon removes excess skin and may also move around and re-position your breast tissue. This procedure creates perkier breasts that look more youthful. If you are satisfied with the size of your breasts, but would like them to be perkier and sit higher on your chest, a breast lift is ideal. When working with Dr. Vennemeyer, you’ll decide on how much lift is right for you.

If you feel that your breasts are both smaller and less perky than you’d like, both procedures combined could be your best route. In a combined procedure, the breasts are moved up and augmented. This combined process is often best when the breasts are smaller than you like and are sitting down further on the chest as well. 1.    Get within 5-10 pounds of your desired, healthy weight to get the best shape and improvement possible with breast and tummy surgery.

2.    DIET CONSIDERATIONS: Significantly reduce or eliminate white sugar and white flour containing products from what you eat. These ingredients are horrible for you, increase inflammation, increase the storage of fat and increase the retention of water. It is incredibly hard to lose weight when you eat any amount of these. Besides, did you know it has been shown that sugar is 8 times more addictive than cocaine? No wonder its so hard to get off the stuff…

3.    EAT RIGHT - Read and follow the eating plan outlined in the book “Eat to Live” by Joel Fuhrman, MD. This is the most enlightened, healthiest, science-based way to eat that I have ever found. People who are stuck at a plateau can lose weight if they actually follow this plan. This way of eating helps detox you and get past the addiction to sugar and refined carbs. You will feel better, reduce your risk of disease and live a lot longer to enjoy the results of your mommy makeover, too!

4.    EXERCISE a little every day (even a 10 minute walk) to maximize health. The better shape you are in, the lower your risk of complications with a mommy makeover procedure.

Factors that influence a successful makeover

Women are happiest after mommy makeovers involving breast lifts when they get really clear on their goals. Specifically, you should consider the following questions:

Do you want your breasts to be bigger and perkier or just perkier?

If you want to be the same size, but perkier, a breast lift alone may do the trick. If you want to have larger, fuller breasts and be perkier, a breast augmentation combined with a breast lift is necessary.

Do you want enhanced fullness in the upper portion of your breasts?

You lose fullness in the upper portion of your breasts when they sag. Although breast lifts move up your nipples and any overhanging tissue, they do not increase the fullness in the upper portion of your breasts much. Sagging, drooping or ptosis means that your nipples or breasts are too low. The inframammary folds or breast creases are used as a reference to determine how much drooping you have. The more your breasts and nipples hang below the inframammary fold, the more severe your sagging is.

Sometimes deflation and sagging are separate issues and sometimes they occur at the same time. Dr. Vennemeyer will determine if you have one or both issues. This is important because different procedures are necessary to get a great result. Breast lifts (mastopexy) improve breast sagging and make breasts perkier by removing extra skin and moving your nipples to the right level.

•    Breast augmentation makes your breasts fuller and larger by adding volume with breast implants. This will not make your breasts perkier!

•    Breast augmentation and breast lift combined with make you fuller and perkier.

If you wish your nipples were higher, you probably need a breast lift. Implants alone will not fix this, but if your nipples are only a little low, many women can get a great result with just breast implants (assuming you want fuller, larger breasts). If you just want larger breasts and you are happy with the level of your nipples, breast implants will likely get you the result you want.

It’s best to be close to your desired weight prior to breast surgery, especially if your breasts change size significantly when your weight fluctuates. If you drop a lot of pounds after cosmetic breast surgery your breasts may deflate to the point where it could affect your result. If you gain weight your breasts may end up larger than you want. Most women maintain a good result when they are within 5 to 10 pounds of their desired weight. Your risks with surgery drop significantly the closer you are to your ideal weight, so the benefits go beyond aesthetic considerations.

Get the right sized breast implants

Breast size is probably the biggest source of satisfaction or dissatisfaction after a breast makeover. Dr. Vennemeyer takes extensive measurements and helps you try on breast implant sizers in the office. This allows you to see what a size looks like on you. You can even try on different outfits to get a sense of what your new proportion would look like in different clothes.  The goal is to help you pick a size you will love.

Get the right breast augmentation procedure

Choose the right procedure(s) to get the result you want. Do not make the mistake of getting large implants thinking it will give you a lift. This leads to big problems. Use the right size of implants to give you the size you want and get a lift if you need to be perkier, too. If you want more fullness in your upper breasts, implants are usually necessary. If you want to be the same size, only perkier, a lift alone may do the trick. However, most of the time women with sagging want to enhance the fullness of the upper part of their breasts and cleavage area. Breast augmentation combined with a breast lift is the most common procedure to achieve this. If you want the fullness, the perkiness, but do not want to be any larger, then a breast reduction with implants can make you perkier, add fullness to the right places and keep you around the same size or smaller.

Can I breastfeed after a breast augmentation?

If you have more children you can most likely breastfeed again, if you choose. The risk of breast augmentation affecting your ability to breastfeed is low.

Will a breast augmentation make my breasts perkier?

Breast augmentation makes your breasts larger and more full, it does not make them perkier. Breast lifts (mastopexy) make breasts perkier by moving your nipples up and reducing or eliminating any breast tissue hanging over your breast creases (inframammary folds). The procedures are often combined to enhance fullness and perkiness.

How can I tell if I need a lift?

o    Perkiness is determined by two factors: the level of your nipples and whether your breasts hang over the inframammary folds.
o    If your nipples and breast tissue are above your inframammary folds, you probably do not need a lift.
o    If your nipples and/or breast tissue hangs below the level of your inframammary folds you probably need a mastopexy to get a good result with breast implants.

Will stretch marks get worse with breast augmentation?

Stretch marks can occur after breast augmentation. They are most common in women who are prone to stretch marks. The risk of breast stretch marks increases when you have tight skin and choose relatively large implants for your breasts. If you already have stretch marks, the appearance of your stretch marks can change when the skin becomes tighter after breast augmentation. You can get an idea of this by pulling a section of skin tight to spread the stretch marks. This makes the stretch marks less obvious in some women and more obvious in others. The presence of stretch marks on your breasts before augmentation does not mean you will get more stretch marks. This is tough to predict and depends mostly on your skin and the size of implants you choose. Larger implants stretch your skin more and increase the risk.

Is there anything that I can put on my breasts to reduce stretch marks?

Unfortunately there is not any product that will eliminate stretch marks. Massaging skin with lotion and giving it time usually results in lightening of any pink or purplish coloration. The over-priced stretch mark products on the market do not do much from what we’ve seen.

Should I get silicone or saline breast implants after pregnancy?

Most moms select silicone breast implants after pregnancy and breastfeeding because they feel much softer than saline implants. Silicone implants also have less wrinkling and rippling than saline. “Gummy bear” breast implants are silicone implants with a thicker gel and tear drop shape that gives a very perky and natural result. These are popular with our patients who want fuller breasts, but do not want to look like they had a breast augmentation.

Any areas of excess fat around the core can be treated with liposuction to improve your shape.
